Growing together.The Field Based Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinant of health such as transportation, housing, and food access.Together, we're removing the barriers that keep people from receiving the kind of quality healthcare that makes a difference. We focus on Integrity, Compassion, Relationships, Innovation and Performance as we empower people to achieve better health and well - being.Schedule: Schedule is Monday through Friday an 8-hour shift to be determined by the hiring manager between the hours of 8 am to 5 pm Eastern Standard Time.Location: Morganton, Marion and surrounding communities in North Carolina, (Burke and McDowell Counties)Local travel up to 75% and mileage is reimbursed at current government rate.Primary Responsibilities:

Linking members to applicable community resources

Complete field visits for high risk, hard to reach members

Telephonic outreach to targeted members

Keep the member out of the hospital by supporting regular visits to their primary physician

Keep member actively engaged with their primary physician

Support the member to ensure pick - up of their Rx

Proactively engage the member to manage their care

Provide member education

Support transitions of care

Create a positive and engaging experience for the member

Help to keep members compliant with their care plans

Collaborate with member’s care team (community, providers, internal staff)

Partner with and support care team by completing delegated tasks

Knowledge and continued learning of community cultures and values

Conduct individual-level screenings to capture member's current health care needs (e.g. Transitions of Care, Health Risk assessments)

Engage member to participate in the screening process and collaboratively develop plan of care based on their individual needs, preferences, and objectives

Performs other duties as assigned

High School Diploma / GED (or higher) OR 5+ years of community outreach experience

1+ years of community outreach and/or field work experience in a health care setting

Intermediate level of computer proficiency including MS Word, Excel and Outlook

Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)

Must have resided within the local community for 2+ years

Valid state of North Carolina driver’s license and state-required insurance with access to reliable transportation that will enable you to travel locally approximately 75% of the time and up to 150 miles round trip to client and/or patient sites within a designated area

Reside in North Carolina within a commutable distance to Morganton, Marion, and surrounding communities (Burke and McDowell Counties)

Preferred Qualifications:

Bachelor's Degree in a health-related field or social work or Health Care Administration

Community Health Worker (CHW) State course completion

Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant

Background in managing populations with complex medical and behavioral needs,

Experience with arranging community resources

Experience with electronic charting

Bilingual – Spanish

Knowledge of Medicaid population

Soft Skills:

Ability to work independently and maintain good judgment and accountability

Demonstrated ability to work well with health care providers

Strong organizational and time management skills

Ability to multi-task and prioritize tasks to meet all deadlines

Ability to work well under pressure in a fast-paced environment

Excellent verbal and written communication skills; ability to speak clearly and concisely, conveying information in a manner that others can understand, as well as ability to understand and interpret information from others
