Mohawk Valley Psychiatric Center is recruiting a Registered Nurse 2 Community to work with individuals with mental illness to prepare them for discharge to a skilled nursing facility. Candidates from outside or within State Service can apply. Minimum Qualifications:Candidates from outside and within State Service may be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as a Registered Nurse 2 Community if they have: License and current registration as a registered professional nurse in New York State, and 2 years of post-licensure clinical nursing experience;1 year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting.Substitution: Bachelor’s degree in nursing can substitute for 1 year of post-licensure clinical nursing experience; and a master’s degree in nursing can substitute for 2 years of post-licensure clinical nursing experience. Neither degree substitutes for the required 1 year of specialized community nursing experience.ORCandidates from within State service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law. Duties Description As Registered Nurse 2 Community at Mohawk Valley Psychiatric Center, you will be part of the Discharge Planning Team as the nurse expert and duties will include: Coordinating meetings with family members to address barriers related to referrals and admissions of individuals to skilled nursing facility Developing and overseeing agreement between Hutchings Psychiatric Center and skilled nursing facility to provide convalescent care Conducting site visits to skilled nursing facilities Coordinating treatment and developing individualized care plans Identifying other supports and personal items to assist the individual living at the skilled nursing facilityNotes:1. If you fail to maintain your license and/or registration once you are appointed, action will be taken to remove you from the position.2. In order to be eligible for appointment and to maintain employment, you cannot be listed as an excluded individual or entity on any of the Federal and/or State Medicaid and Medicare exclusion lists (or excluded from any other Federal or Federally assisted program). If you are appointed and subsequently listed as an excluded individual or entity on any of these lists (or excluded from any other Federal or Federally assisted program), you may be terminated from your employment.3. If appointed you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES); and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid. If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
