Req Number 144100Job DescriptionLocates, sorts, delivers and files patient medical records in support of ambulatory patient care setting department.Job Responsibility1.Identifies, locates and pulls medical records for patients scheduled visits.
Pulls charts for telephone call contacts as requested by clinical team members.
Files patient test results within established timeframe.
Maintains file of patient test results not filed due to unavailable medical record.Continually seeks out medical record.
Sorts patient test results by requesting physician and distributes accordingly.Makes additional copies, as requested.
Purges department medical records according to established policies and procedures (i.e inactive, deceased, etc).
Serve as a medical record liaison to requesters with correspondence needs.
Processes responses to correspondence requests by providing medical record copies to attorneys, insurance companies, physicians and NYS Disability, etc.Sends record copying requests to correspondence staff.
Maintains filing areas and chart order/appearance according to department standards.
Reviews department medical records to ensure adherence to standards mandated by Department of Health and The Joint Commission, as requested by management.
Creates new patient folders, as needed.
Returns records to file, separating records for patients with upcoming scheduled return visits.
