SECTION I BASIC FUNCTION: The Supervisor of Case Management and Social Work (CMSW) assists with the daily operations of the Department. This person can assist with the hiring and managing of all the CMSW department staff. One must possess the ability to assess and reasonably manage variables contributing to the reduction of length of stay (LOS) and cost per case through resource management. The Supervisor of CMSWS is responsible for assisting with all daily operations of the department and provides coverage in Managers Absence. This person in this position will ensure clinical documentation adequately reflects resource management and severity of illness of the patient population(s). The Supervisor of CMSWS assist with providing the link between compliance, clinical management, patient progression, gathering and reporting of utilization data, community outreach and as a resource in the prevention of readmissions. SECTION II EDUCATION AND EXPERIENCE REQUIREMENT(S):

Active NYS RN Licensure or master’s in social work with active Licensure

Previous UR, Discharge Planning, Case Management, or post-acute discharge planning experience.

Eligible for PRI or Screen certification.

Eligible for or certified as: Certified Case Manager

Proven track record of self-directed, independent practice and the ability to coordinate and work with multiple processes and people successfully

1-2 years supervisory experience preferred

SECTION IV

PHYSICAL REQUIREMENTS:

Should be able to push/pull, lift/ move 15lb., be able to perform moderately difficult manual manipulations such as using a keyboard, writing, and filing for extended periods of time, must be able to perform tasks which require hand-eye coordination such as data entry, typing and using photo copiers. Mobility requirements may include the ability to be stationary at a workstation for a prolonged period time in addition to being able to squat or be mobile for a reasonable length of time and distance. Communication requirements include the ability to comprehend the spoken English language in addition to being able to communicate and read the English language.

SECTION V

PRIMARY RESPONSIBILITIES OF THE POSITION:

Direct oversight of the daily operations and functions of the CMSWS department

Coordinate CMSWS Department Staff schedule

Assist with payroll for the CMSW Department

Assist with the completion of annual performance evaluations for the CMSW Department

Oversight of orientation and continuing education for staff

Assists with developing policies, protocols, and procedures

Attends departmental and organizational meetings and trainings as necessary

Acts as a role model for a high standard of case management practice and performance

Assist with ensuring that all human resources procedures are followed including hiring, discipline, evaluations, and grievances

Adheres to hospital policies and procedures

Administrative component of Healthcare Resource Management and hospital-wide Quality Improvement program

Provides guidance, direction, support, and the environment that enables the staff and our customers to adhere to their responsibility

Assist Manager with HR related processes

Utilization of data analytics to direct efforts on key performance indicators including progression of care, length of stay, quality indicators and alike

Provides coverage of Managers responsibilities when Manager is not available.

Other duties as required
