ResponsibilitiesPosition Summary:UR Medicine Employee Wellness provides comprehensive services across western New York. This nurse will work in Passport Health satellite office in Syracuse providing travel medicine services, biometric screening services in the Syracuse area, and telemedicine coaching. This role focuses on providing the highest quality direct care services, enhancing access to URCEW services and assuring an excellent customer experience. The nurse will work collaboratively with the clinical and administrative leadership in School of Nursing to assure efficiency of service delivery and coordination of services in multiple settings.Typical Duties and Responsibilities:

Provide comprehensive travel care services including individualized recommendations and advice based on the client’s health history, travel itinerary and best practice standards. Use technology and resources to augment service.

Administer required and recommended vaccines based on the client’s needs/ health profile using standing orders as appropriate. Conduct mobile flu clinics.

Participate in CEW programs including mobile biometric screening clinics, coaching, group programs and community wellness. Conducts assessments, plans and evaluates care using culturally sensitive approaches based on current best practice standards and available evidence. Assures accurate and complete documentation of client records, including goals, coaching interactions, outcomes, and referrals.

Conduct and coordinate intake assessments, ongoing appointments and program completions for clients enrolled in a condition management or lifestyle management program.

Maintain competency in current evidence within and across programs and apply to each client encounter. Comply with all regulations and standards (e.g. documentation, NYS regulations) as required.

Collaborate with team members, including the PPH medical director and Chief Wellness Officer, to assure safe, efficient, and effective care. Travel to Rochester office periodically as requested to promote collaboration.

Other duties as assigned: Participate in QA, productivity reviews; participate in professional self-development; Maintain clinic standards per regulation, directive, professional standards.

Qualifications:

Bachelor's degree in nursing or health related field required

RN - Registered Nurse - State Licensure and/or Compact State Licensure upon hire required

BS preferred

2 years of relevant experience required or equivalent combination of education and experience required

5 year’s experience in health care. Wellness background a plus

Excellent clinical skills, communication skills. Must be able to work independently
