Job DescriptionPride Health is hiring a Licensed Practical Nurse to support our client’s medical facility in Rochester, NY. This is a 13-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization!Job Summary :We are seeking a compassionate and dedicated Licensed Practical Nurse to provide high-quality nursing care to patients in a nursing home facility. As an LPN, you will work closely with registered nurses (RNs), physicians, and other healthcare professionals to deliver effective and patient-centered care.Key Responsibilities :Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rate.Administer prescribed medications, document patient reactions, and report any changes in patient condition.Assist in dressing changes, wound care, and other necessary procedures under the supervision of a registered nurse or physician.Help patients with activities of daily living (ADLs) such as bathing, dressing, grooming, eating, and mobility.Assist with medical procedures, such as catheterizations, injections, and IVs, as directed by the RN or physician.Educate patients and families about health conditions, medications, and post-discharge care.Maintain accurate and up-to-date patient records, ensuring HIPAA and confidentiality standards compliance.Collaborate with RNs, physicians, and other healthcare providers to develop and implement patient care plans.Track changes in patient condition and report any significant changes to the healthcare team.Ensure a clean, safe, and comfortable environment for patients.
