If this description speaks to you, continue reading about the opportunity of Financial Liaison. This is a full-time, 8:30am-5:00pm 40 hours per week position at St. John’s Home. Position Summary: The Financial Liaison is responsible for reviewing and making determinations on the financial status of potential incoming or re-admitting residents, as well as residents seeking to transition from rehab to long-term care in a skilled nursing facility. Duties include verification of insurances, obtaining prior authorizations, discussions with families, attorneys, case workers and financial case managers regarding insurances, assets, transfers, properties, private payment and Medicaid. Effective communication with all other departments to coordinate placement, manage beds, and share updates on payer status. Assistance with admission agreements, inquiries and tours as needed. Responsibilities include:

Work closely with Admissions, Business Office and families to evaluate and forecast payer status and identify potential barriers for new admissions, re-admissions and existing residents.

Complete insurance verification for all new admissions and re-admissions, acquire pre-authorizations as needed; communicate information to St. John’s Home staff, families and residents relative to coverage/non-coverage.

Schedule appointments with newly admitted residents and/or families who have questions regarding Part D, Rep Payer, Medicare, Medicaid, insurances and private payment.

Timely issuance of cut letters/denials to families and share with other departments as needed.

Review and sign financial forms and Admissions Agreements with families/financial representatives.

Evaluate applications for long-term care from rehab residents as they are sent along by Rehab Social Workers.

Slot residents for rehab and LTC beds as appropriate, per payer allowances.

Set up appointments and conduct facility tours for perspective applicants/families.

Provide support to new admissions coming into St. John’s. Inspect and coordinate rooms for occupancy including: coordination with building services, environmental services and neighborhood teams.

Responsible for creating resident records and updating EMR with accurate insurance information in compliance with audit regulations, including accurate bed hold status for hospitalized residents or those on social leaves.

Assist with logging rejected/withdrawn referrals; maintain data base regarding disposition of all referrals that have been rejected/withdrawn.

Maintains fax machines, monitors supplies and contacts vendors for problems, as needed.

Develop relationships with assigned neighborhoods and ensure that constant communication occurs regarding census, updates, and hot spots.

Qualifications:

Bachelor’s Degree in Social Work, Human Services or related field.

Minimum of three years’ experience in skilled nursing facilities, familiarity with the admissions process, census management and payer mix.

Experience with Medicaid, Medicare, private payment and third party insurances.

Ability to prioritize and manage multiple tasks, often with interruptions, in a fast-paced office environment.

Physical Requirements:

Occasional moving of Elders.
