Req Number 120822Job Description. Outpatient: Scheduling and maintaining daily schedule. Physician initial order or referral. Diagnosis for therapy services including date of onset; initial evaluations and all re-evaluation in accordance to Medicare and Insurance guidelines. Plan of treatments with MD signature pertaining to claim period. Progress notes and attendance records pertaining to the claim period to support total minutes of actual treatments and number of minutes for each modality Clinical progress notes. Perform videlfluroscopic swallowing studies. 2. Hospital: Patient education sheet. Multidisciplinary care plan. Education for nursing staff and PCTs. Updated forms and policy. Perform standardized patient testing on each patient when appropriates. Perform videofluoroscopic swallowing studies.Job Qualification

Master's Degree in Speech Pathology, required. Currently licensed in Speech Pathology in the State of New York. Certificate of Clinical Competency from the American Speech-Language-Hearing Association, required. Eligibility for certification in Speech Pathology by American Speech and Hearing Association.
