Job Description Summary:Job Description:Embedded within the Moses/Weitzman Health System, America’s first health system dedicated to primary care for underserved populations, the Weitzman Institute is dedicated to promoting equity and optimizing health outcomes for vulnerable populations through innovative research, education and policy. Since 2007, the Institute has grown into a nationally recognized organization with expertise in social determinants of health, workforce development, and systems transformation. Its unique environment, within a safety-net health system, has enhanced its ability to conduct research and promote innovation aimed at improving outcomes, decreasing health disparities, and developing implementation strategies for evidence-based, integrated practice for federally qualified health centers and other primary care practices nationwide. The Institute has worked with practices in all 50 states, focused on building provider capacity and overcoming provider isolation in 15 content areas highly relevant to primary care, including complex integrated pediatric care, trauma-informed care, pain management, substance use disorders, HIV and HCV treatment, COVID-19 care, LGBT Health, diabetes and health weight management, as well as other complex conditions.The Project Manager will support a select portfolio of initiatives involving cross-collaboration across the affiliates of MWHS. The primary project the Project Manager will support is the Heal Our Youth intervention, an evidence- and strengths-based mindfulness and restorative justice training program aimed at promoting physical and emotional well-being, as well as addressing community violence and trauma among vulnerable youth in New Britain, Connecticut. Heal Our Youth is a collaborative program among the Weitzman Institute, Community Health Center, Inc., and ActivePlus, a Black-founded and operated nonprofit that developed the Heal Our Youth intervention, funded by the Singer Foundation.Equity & Inclusion is a mission-critical component of the culture at the Weitzman Institute. The Project Manager should be in alignment with this core value and demonstrate commitment through their work by respecting different cultures of patients, clinics, and team members.Responsibilities:

Oversee the day-to-day operations of assigned interdisciplinary projects, including a strategic initiative focused on clinical-community partnerships to support vulnerable youth disproportionately at risk of exposure to community violence and trauma

Assume day-to-day coordination, communication, risk mitigation, and relationship management for assigned projects

Develop and maintain communications with key stakeholders (e.g., senior administrative and clinical leaders from CHCI who have a nuanced understanding of and longstanding relationships with youth, families, and communities in New Britain; ActivePlus, developer and implementer of Heal Our Youth intervention) through frequent virtual and in-person check-ins and email correspondences

Assume responsibility for the implementation and quality of all program components, including direct in-person oversight of both the participant recruitment process, as well as participant engagement in the intervention

Coordinate cross-project activities, including monitoring project milestones and reporting

Engage with subject matter experts on innovative and interactive approaches for program content delivery

Provide support to Principal Investigator in managing administrative and financial elements of assigned projects, including preparation of materials for Institutional Review Board (IRB), monitoring of progress towards achievement of key project milestones, and development of interim and final reports for funders and other key stakeholders

Support PI and key collaborators in the development and submission of materials (e.g., conference abstracts, peer-reviewed publications, policy briefs) to promote widespread dissemination of key findings from initiative

Represent Weitzman Institute at national, state, and local conferences and meetings as needed

Requirements:

Work experience must demonstrate track record in project management, adaptability, flexibility, accountability, teamwork skills, and ability to meet deadlines in a timely manner

Excellent interpersonal, written and oral communications skills

Strong relationship development skills with problem solving techniques

Ability to work independently and collaboratively

Proven experience and ability working in matrix organizations

Proven experience working with clinical, community based organizations, and/or other institutions serving vulnerable youth

Ability to work in a hybrid environment and travel throughout New Britain, CT: This position requires being on site at CHCI New Britain location 3x/week (frequency adjusted based on needs of the program) to conduct participant recruitment and oversee implementation of the intervention

Additional Specifications:

Bachelor’s degree required, Master’s degree in a related field preferred

At least 2 years of related work experience

Bilingual is a plus but not a requirement
