Leads a team of Personal Support Workers to provide qualified personal care to patients in a home settingTravel to patient’s homes in community and facility settingsWork collaboratively with multidisciplinary teams in community and facility settingsAssesses patient needs, according to the timeframe established by contractual agreements, formulates the plan of care and assesses progress towards outcomesResponsible for the management of staff, conducting performance appraisals and implementing progressive disciplinary actionGathers, records and analyzes patient and employee feedback and maintains an awareness of key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion ratesEnsures that all patient concerns and complaints are addressed and documented in a timely mannerPromotes Spectrum Health Care through participation in local healthcare or business committees as well as community eventsParticipates in ongoing internal and or external continuing education for personal development
