We are seeking a Social Worker/ Case Manager to join our team! The ideal candidate will provide social work support to frail elderly clients in Brooklyn Full time M-F 9-5. No on-call work. Bilingual in Spanish, Creole a plus, but not necessary. Hybrid schedule 40% remote work, 60% in person.

Responsibilities:
Conduct comprehensive needs ( Bio-psychosocial ) assessments, develop and implement care plans based on identified needs. Conduct regular follow-up to ensure that participants' needs are being met. Meet with family members, caregivers, and others who play a key role in the participants' lives as dictated by the care plan.
Identify and use appropriate resources to respond to participant needs and make any referrals necessary to implement care plans. Provide follow-up with clients and collateral contacts as indicated.
Input data and maintain comprehensive electronic and paper case files, including intakes, care plans, case progress notes, event participation, and units of service provided, and other important documents.
Liaise with partners and community groups.
Monitor clients via telephone and home visits
Access outside resources and/or programs to facilitate all unmet needs of clients
Assess nature of elderly clients' situations by interviewing and reviewing personal history and current milieu
Develop and execute individual treatment plans
Determine appropriate milestones to gauge client progress and determine future needs
Communicate client updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs
Maintain accurate client documentation
Qualifications:
(L)MSW
Masters level w/ 1 yr experience in human services
BSW or BA/BS ( in field closely related to SW)

Demonstrated case work experience
Demonstrated experience with aging population, preferred.
Understanding of the aging process and its impact on individuals physically, socially, mentally, etc.
Demonstrated experience with working as a member of an interdisciplinary team.
Experience with volunteer activities a plus.
Excellent organizational, verbal, written, and interpersonal skills.
Strong problem solving skills with the ability to effectively handle crisis situations
Excellent computer skills.
Demonstrated ability to perform multiple tasks effectively in a fast-paced, challenging, and constantly changing environment.
Bilingual a plus
