Concern Housing is seeking a Program Supervisor to be responsible for the overall supervision and management of program site to help promote tenant growth, safety, and the effective delivery of support services. The role also includes support to the Concern Pitkin site as a back up to the Program Director.
Position: Program Supervisor
Location: Brooklyn, NY (Pitkin Ave. Logan)
Schedule: Tuesday - Saturday; 8am - 4pm
Salary: $68,000/yr.
Duties and Responsibilities:
- Assist with the supervision and coordination of all program services, including staff scheduling, supervision and crisis intervention
where needed (including 24/7 cell phone coverage).
- Oversee all intake procedures in conjunction with NYC Admissions Committee, including screening of applicants and monthly
outcome reporting to HRA CAPS. Maintain program occupancy rates (>95%).
- Compliance with OMH, LIHTC and HHAP regulations, including eligibility determination, annual certifications and any other
reporting requirements, as necessary. Assistance with all monitoring visits.
- Monitoring client charts for timely submission of electronic chartwork in Precision Care, and qualitative review of progress notes,
support plans and reviews, functional assessments and other required documentation in accordance with agency QI/ QA Compliance
schedule.
- Cooperation with the Property Management department re: site maintenance and safety issues, as well as the accounting department to
ensure monitoring of rental payment/ arrears and work closely with NYC Financial Case Manager in assisting tenants with securing
and maintaining entitlements.
- Provide staff training and development to ensure suitable implementation and evaluation of evidence-based best practices. Evaluate
staff performance and promote professional growth
- Collaboration with local hospitals and healthcare/ care coordination providers through committees, special projects, etc., as well as
community outreach.
- Provide program support to Concern Pitkin program in consultation with Pitkin Program Director
- Other tasks as assigned by the Program Director.
Requirements: Master’s Degree preferred in Psychology, Social Work, Nursing or related mental health field with at least 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good leadership and supervisory skills, good verbal and written communication skills and the ability to make sound judgments regarding client care. Residential experience a plus. CASAC preferred.
