Eprine Home Care is seeking a Patient Coordinator. This position requires strong interpersonal skills and ability work as a team member, ability to travel to patient’s homes and provide aide supervision: Working closely with Nurses: AAS and BA required
Job summary
Strong writing and verbal and interpersonal communication skills
Experience with coordinating home care
Basic knowledge of Microsoft Word and Excel and HHA Exchange
Excellent organizational and multi-tasking skills
Previous experience in a home care managerial role
Good Time Management and project management skills
Match caregivers from our large employee pool with clients in need of services
Follow up with clients by phone to ensure high levels of service are provided, escalating any concerns to your supervisor
Problem solve challenging client schedules
Monitor caregiver performance, escalating any concerns
Must be willing to be on call
Must be willing to travel and Must have a driver’s license
