Job Title: Community Habilitation

Staff Job Summary:

As a Community Habilitation Staff, you’ll empower individuals with developmental disabilities to achieve greater independence and community integration. This role involves personalized support, aiming to enhance participants’ social, financial, and practical skills for a more independent life.

Key Responsibilities:

1. Personalized Support: Tailor assistance to meet individuals’ unique needs and goals, enhancing their autonomy.

2. Goal Development: Work with participants, their families, and support teams to set and achieve skill-building and independence goals.

3. Community Integration: Facilitate access to and participation in community resources and activities.

4. Skill Development: Instruct in essential skills for daily life, social engagement, and community involvement, such as financial literacy and household management.

5. Documentation: Keep accurate and timely records of daily activities, monthly summaries, and notable incidents, adhering to regulations.

6. Communication: Ensure ongoing communication with participants, their families, and support teams, updating them on progress.

7. Advocacy: Champion participants’ rights and needs, securing their access to community services and supports.

Qualifications:

High school diploma or equivalent required; a degree in social work, psychology, special education, or related field preferred.

Experience with individuals with disabilities desired.

Clean/Valid driver’s license and reliable transportation required.

Background check and state-specific certifications must be passed.
