Case Manager

The Case Manager promotes client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation.

RESPONSIBILITIES:

Caseload of approximately 25 families.

Meets with caseload participants weekly and as needed on an individual basis, through assessment with Director.

Links participants with appropriate providers and resources throughout the continuum of health and human services and care settings based on the needs and values of the participants. Escorts participants to scheduled and unscheduled psychiatric/medical care.

Helps to navigate the public assistance system and other city agencies. Ensures families’ cash assistance benefits are active.

Ensures that participants receive safe, effective, client-centered, timely, efficient, and equitable care. Maintains a non-judgmental, unbiased approach.

Prioritizes best practices for participant safety and health in the face of immediate crises, through the outreach of qualified emergency responders.

Conduct unit inspections and ensure proper follow-up to findings, assisting participants as needed.

Maintains participant records as “audit-ready” at all times, as required by program funding sources. This includes, but is not limited to, recording progress notes, constructing independent living plans, updating medical and mental health information, and completing psychosocial assessments.

Adheres to confidentiality protocols outlined by the agency.

Works alongside Housing Specialist to determine best exit strategy based on participant individual needs and desires.

Collaborates with ACS when a family is child welfare involved.

Performs other duties as assigned by the Director.

EDUCATION:

Bachelor’s Degree preferred.

EXPERIENCE:

Minimum of 2 years in human services

Working experience with families and knowledge of issues related to homelessness, HIV, substance abuse, mental illness, and entitlements.

SKILLS & ABILITIES:

Good spoken and written expression.

Good time management skills, able to handle multiple tasks and assignments at once as well as adhere to the punctuality of a set schedule.

Sensitive to intersectionality, different cultures, housing equality and a demonstrated ability to work within a social/racial justice framework.

Working knowledge of Microsoft Office (Word and Excel).

Knowledge of CARES system a plus.

Bilingual preferred.

OTHER REQUIREMENTS:

Crisis intervention skills.

Familiarity with Health Insurance Portability and Accountability Act (HIPAA).

Willing to participate in continuing education.

This is temp-perm, temp for 500 hours, then a salary and benefit package.
