We are seeking a dedicated part time Registered Nurse (RN) to join our team of compassionate healthcare professionals.
All County Health Care Referral Services of New York is dedicated to providing quality, compassionate and supportive services in an ethical manner to home care patients. These services performed with dignity and respect will be available to individuals of all ages regardless of race, creed, disability or national origin.
RESPONSIBILITIES
Job duties consist of, but are not restricted to:
Nursing visits (in home)
Assess and chart observations of the patient’s condition at each visit.
Monitor vital signs, symptoms, and any changes in the patient's condition.
Home safety assessment according to patient needs.
Complete evaluation tasks, including reviewing medication.
Administer Physician-prescribed medication.
Dress or redress wounds and assess the healing progress.
Provide education to patients and families on proper home health care procedures and strategies.
Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan.
Keep the patient comfortable through palliative care.
Documentation and Reporting.
Recommend tools or devices that may improve the quality of life for the patient.
Qualifications:
Valid RN license in New York.
Minimum 1years of experience in clinical nursing, preferably in home health or a related field.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Reliable transportation and willingness to travel to patients' homes.

PAY RANGE: $50.00-$100.00 per hour depending on experience.
