Positions & Qualifications:
Care Manager: (Field Visits) Manhattan
Payrate: $23.17 - $28.96 Hourly
Overview:
Conducts assessments and develops client/member centered plans of care. Provides coordination of services between the varying providers for clients / members with complex psychiatric, substance use, and/or co-morbid medical conditions. Ensures access and linkage to the full array of necessary physical and behavioral health services and other community-based services to address social determinants of health. Coordinates effective communication between all providers for the ultimate benefit of the client/member. Works under general direction.
Education:
Bachelor's Degree in a human services or related field required Enrollment
Behavioral Health Associate: (Field Visits):Brooklyn
Payrate: $25.46 - $31.86 Hourly
Overview:
Provides direct social work services in collaboration with the Licensed Social Worker and the team consistent with VNS Home Care policy. Works under moderate supervision.
Qualifications
Education:
-Bachelor's Degree in Social Work or a related human services field required
Work Experience:
-Minimum one year of social work experience in a mental health or social service setting required
Family Advocate: (Field Visits)- (Bronx), (Brooklyn), (Queens)

Overview: Provides advocacy, case management and family support services to children and their families. Assists with entitlements, linkages, and collaborates and advocates with systems involved with the client. Works under general direction.
Education: -High School Diploma or the equivalent required
Licenses and Certifications:
-Valid driver’s license (i.e., license matching state of residence) required
-Credentialed family peer advocate preferred
-Employer will provide vehicle to staff members
Wellness Health Coach- Brooklyn & Manhattan- (Field Visits)
Payrate: $19.18 - $23.99 Hourly
Overview:
Provides, education, client advocacy, regular evaluation and feedback about clients on caseload. Also evaluates clients' eligibility for other social service programs. Alerts team members when follow-up is required and ensures efficient and successful access and linkage to the full array of community based services with the goal of client's successful completion of the care plan. Works under close supervision.
Education:
High School Diploma or equivalent required associate degree or bachelor's degree in a human services or related field preferred
