JOB DESCRIPTIONThe Home Infusion Clinical Services Liaison – RN, is responsible for:

Positive and effective representation of Mount Sinai Home Infusion and introducing the full scope of services offered to hospital case managers, physicians, provider staff, nursing agencies, other referral sources, and patients and their caregivers.

Providing education about the program and services to referral sources and patients.

Assisting with clinical coordination of care between referral sources, the pharmacy, nursing agency and patient / patient caregivers, on behalf of Mount Sinai Home Infusion.

Providing and utilizing clinical expertise and skills to assist referral sources &/or patients with:

The identification of appropriate candidates for home infusion

Confirming the patients’ and/or caregivers ability to receive care in the home

Patient education and training regarding therapy and supplies, as well as, clarifying expectations of care in the home.

Medication connection at the hospital / facility in order to expedite the discharge into the home setting, as appropriate

RESPONSIBILITIES

Develops, maintains and grows core therapy and service line admissions to The Home Infusion Center through positive referral source relationships and responsive, caring customer service.

In coordination with the General Manager, identifies and communicates opportunities for expanding the home infusion business among current referral sources and new opportunities.

Demonstrates thorough knowledge of core therapy referrals and qualifying criteria by payer source, such as: TPN, Enteral Nutrition and Inotropic Agents for Medicare.

Evaluates current and future clinical and service needs for existing and new service areas.

As appropriate &/or required, coordinates &/or conducts in-services and continuing education programs for referral sources and their support staff.

Collaborates with referral sources (i.e. hospital case managers, home health agencies, physician offices) and Mount Sinai Home Infusion staff to facilitate the coordination of care for new patient referrals to Mount Sinai Home Infusion.

Coordinates and communicates all critical workflow activities for new patient referrals to Mount Sinai Home infusion personnel and referral sources, including, but not limited to:

Intake

Clinical Operations: Pharmacy &/or Nursing

Warehouse Operations, if required

Reimbursement, if required

Appropriately documents the coordination of care activity including, but not limited to:

Home infusion &/or nursing services utilized

All communication with patients, referral sources, physicians and Mount Sinai Home Infusion personnel

All patient &/or caregiver education & training.

Ensures all documentation is legible, accurate and complete and complies with company policies & procedures, as well as regulatory and accreditation standards.

Maintains up-to-date referral source records, as well as, any other requested records in accordance with Mount Sinai Home Infusion’s policies, procedures &/or business goals.

Assist with activities to achieve favorable net revenue and gross profit goals for Mount Sinai Home Infusion.

Represents Mount Sinai Home Infusion in a professional and positive manner.

Attend and participate in appropriate office meetings, sales meetings, training programs and regional and national trade shows and conventions as directed by the General Manager

Coordination and cross-training of departmental work duties to achieve maximum productivity and efficiency.

Stay abreast of developments in field of infusion therapy.

Comply with all company policies and directives for the fulfillment of the company objectives

Communicates any problems incurred directly to The General Manager

QUALIFICATIONSEducation RequirementsBachelor’s degree from an accredited college or school of nursing.

Strongly Preferred:

3 years of IV related experience

2 years of Home health care experience

1 year of Case Management experience

Sales and Customer Service experience

Must have and maintain current CPR certification

CRNI, preferred

The salary range for this role is $87,692.00 - $131,538.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need.
