This accomplishment requires a steadfast and daily commitment to maintaining the Department's core values.The Fire Department, City of New York (FDNY), seeks a full-time Case Management Nurse in the Bureau of Health Services, World Trade Center Program located at Staten Island. Reporting directly to the WTC Nursing Supervisor the successful candidate will: maintain a comprehensive case management system for the WTC Oncology Program in the FDNY WTC Medical Monitoring and Treatment Program for active and retired members of the FDNY. Responsible for case management, including but not limited to, screening appointments; tracking and receiving test results; entering results into EMR; perform utilization/concurrent review on all screening tests as well as on all inpatients; assessing discharge planning needs at admission. Coordinate all home care services; placement and transfers and facilitate a safe discharge plan. Provide outpatient assessment and management of a caseload of ambulatory patients, including follow-up of clients, collaboration with and accountability to physicians for health and medical status assessments of patients. Document all client encounters, including nursing summaries. Evaluate and document the progress of individual screening and subsequent diagnostic testing required as well as the progress of individual therapeutic programs and make modifications, as required. Confer with physicians in developing initial screening, diagnostic evaluations and initial treatment plans, report progress, and analyze changing symptoms and health trends. Coordinate initial assessments and ongoing reassessments of the client’s functional status.Qualifications

A valid New York State license as a Registered Nurse, plus a baccalaureate degree in nursing from an accredited college, and three years of clinical nursing experience; or

A valid New York State license as a Registered Nurse, plus an associate degree in nursing from an accredited college, and five years of clinical nursing experience; or

Education and/or experience equivalent to '1' or 2’ above. However, all candidates must have a valid New York State license as a Registered Nurse.
