Under the clinical direction of the Coordinator of Community Nursing and the administrative direction of the Director of Professional Services. The Day Program Community RN/ AMAP Instructor ensures that health and nursing quality standards are maintained for the individuals who reside in a Richmond Community Services residence. This includes providing direction to the direct care staff, performing direct nursing care, and coordinating outside medical services. Teaching AMAP classes and conducting medication pourings as needed at DPN and the community.Day Program Community RN/AMAP Instructor demonstrates knowledge of Richmond's quality mission, vision, and values. In addition, this position actively participates in quality improvement projects (i.e. communicating issues, offering suggestions) to ensure ongoing excellence of Richmond residents, staff, and programs to maximize the quality of life for individuals suooorted by RCS.Essential Job Functions:

Renders direct nursing care by examining individuals' conditions.

Carries out person -centered health related supports and services in a respectful and positive manner.

Adheres to medical and nursing routines, urgent care and emergency care procedures in accordance with DOH, OPWDD and Richmond's medical and nursing policies and protocols.

Conduct AMAP training and medication pourings at DPN and in the community.

GER assessments and follow-up with documentation in Therap. lnservice residential shared staff at DPN as needed. Conduct meal observations weekly. Update Nursing reports as needed. Attends Telehealth visits requiring medication changes, or changes to care plans. Communicate with residence nurse via email, phone and documentation in Therap.

Contacts the Physician, when necessary, by telephone and/or in writing to discuss changes in individuals' conditions, results of consultations, lab tests, etc.

Follows agency requirements for medication ordering, storage, inventory, and disposal. Transcribes and follows through on all Physician orders.

Oversees the administration and accurate count and control of all controlled drugs and ensures proper control of syringes, according to the instructions of the Bureau of Narcotics and State Bureau of Hospital Services. Prepares, administers and records medications and treatments to individuals as needed in accordance with standard nursing practices.

Participates in the planning meetings (Monthly Clinical Meetings, Annual and Semi­-Annual meetings), responsible for implementation and evaluation of the plan of care for each individual.

Ensures that all necessary health documentation is accurate and are up to date (clinic visits, inoculations, quarterly and annual reviews, etc.)

Ensures that all equipment is in good repair and functions properly. Informs Residence Manager of equipment needs.

Provides ongoing direction and feedback regarding the performance of direct support staff and reports issues as needed to the Residence Manager according to Richmond policies.

Acts as a liaison between other community agencies (schools, programs, clinics, families, etc.).

Observe medical needs of program participants and compile all relevant data into a medical file/chart.

Conducts nursing in-services annually or as often as necessary based on observed training needs.

Identifies training and development needs of staff and communicates this information to Residence Manager and appropriate Workforce Development personnel verbally and/or in writing. Provides observation. of staff following in­service training or re-certification and reports findings to the RN.

Meets with the Community Nurse Coordinator, routinely to resolve issues and communicate relevant information.

In conjunction with the Community Nurse Coordinator, keeps current with nursing policies, practices, and regulations by attending seminars, conferences, and in­service training.

Qualifications & Minimum Requirements: Graduate of an accredited school of nursing Current New York State RN license.
