Substance Use Disorders are chronic illnesses, and treatment provided is based upon the presenting needs of the individual. Our goal is to help individuals and families begin the physical, emotional and spiritual healing process by utilizing compassionate, encouraging and evidence-based practices. We ultimately strive to guide individuals, and their families, towards a goal of living life to its full potential-free from addiction. We are currently seeking a part-time Dually Certified Psychiatric & Family/Adult Nurse Practitioner licensed in NY State. The Dually Certified Psychiatric & Family/Adult Nurse Practitioner will provide comprehensive evaluation and treatment to adult patients with substance use disorders and mental health diagnoses in a short-term detox and rehab setting. Assess appropriateness of admission for detoxification and rehabilitation and level of care. Provide primary treatment of acute and chronic medical and mental health conditions and triage to higher level of care as needed. Determine ongoing ability to participate in a medically supervised non–psychiatric setting. The Dually Certified Psychiatric & Family/Adult Nurse Practitioner will be supported and work collaboratively with the Clinical & Medical Providers under the direction of the Medical Director. Responsibilities:
Conduct evaluation with relevant history, health assessment based on the patient’s signs & symptoms.
Identify needs of the patient and make a differential diagnosis based on the clinical presentation, history and clinical findings.
Perform psychiatric evaluations on initial patient visit and psychiatric assessments on follow-up visits.Must be able to obtain complete medical, psychiatric, and psycho-social history.
Manage/triage psychiatric emergencies.
Formulate differential diagnoses in accordance with DSM-5 criteria, facilitate appropriate treatment plans and prescribe medication for treatment of patients with co morbid substance use disorder and mental, emotional, and behavioral disorders.
Provide psycho-pharmaceutical medicine management and counseling for appropriate patients.
Conduct waived testing for urine, blood work and glucose monitoring.
Develop a prioritized and mutually acceptable plan of care according to the written practice protocols in collaboration with the physician.
Implement a treatment plan that is prioritized and mutually acceptable with appropriate diagnostic tests, pharmacological and non-pharmacological interventions, patient education, and with appropriate referrals as needed.
Provide systematic follow-up by evaluating the effectiveness of various treatments provided, therapeutic processes used, their outcome and modifying treatments & management plans as necessary to achieve the optimum mental health goals.
Provide educational materials & information needed to make well informed choices and decisions by patients about their health promotion, illness prevention and finally, illness treatment.
Facilitate the patient’s entry into the primary and mental health care system with appropriate utilization of quality and cost-effective health care services.
Provide a safe environment for the patient, and promote patient advocacy at the individual and community levels
Participate with other professional colleagues to provide comprehensive physical and mental health care.
Maintain accurate, legible and confidential medical records.
Requirements:
· Master's & PMHNP Master's or doctoral degree · Currently registered as a New York State Registered Nurse Practitioner (NP) and Psychiatric Nurse Practitioner (PNP) license · NP & PNP license are in good standing · Certified as a nurse practitioner pursuant to§6910, with prescription writing privileges pursuant to§6902.3b of the Education Law · DEA license · OPRA Certified · Hold all requirements indicated by the NY Department of Education and NYS OASAS to perform history and physicals and prescribe medications · Current basic life support for health providers (BLS) · Two (2) years of related work experience · ANCC credentialing and certification · Evidence of prior/current collaborative agreement (as applicable) with a NYS licensed Psychiatrist
