Concern Housing is seeking a Senior Case Manager who will be responsible for providing case management services directly to clients. The Senior Case Manager is considered a resource in that they should be familiar with community resources and services to which they may help their clients gain access. The Senior Case Manager also acts as a resource for CTAP case managers and supports the Director as needed.

Position: Senior Case Manager

Location: Medford, NY

Schedule: Schedule Options: Tuesday-Saturday; 9am-5pm or Sunday-Thursday 9am-5pm

Salary: $55,000/yr.

Duties and Responsibilities:

Provide case management services (service planning, medication monitoring, assistance in forming linkages and/or referrals to clinical services, day programs, vocational services and opportunities, community integration, entitlements etc.)

Provide individual counseling and support services; responsible for all documentation.

Oversee housing services, including assistance with maintenance issues.

Provide site visits as needed and directed.

Perform monthly fire safety and unit inspections; complete weekly smoke detector checks and document as required.

With the Director, ensure compliance with regulatory guidelines with regard to OMH, Medicaid, and CARF.

Attend weekly supervision and staff meetings.

Perform a quarterly review of Face Sheets to ensure accuracy of the Emergency Binder.

Assist with orienting new staff to CTAP, including:

Initiate necessary training (Precision Care, CTAP procedures, etc.)

Coordinate initial site visits with case managers

Monitor the completion of the New Hire Orientation Checklist

Participate in the Intake process, as scheduling permits.

Support the admission process for new clients, including working to secure necessary documentation, etc.

Facilitate case manager meetings, as scheduling permits.

Remain current with all Agency required compliance documents and trainings.

Other tasks as assigned by the Director or Program Coordinator.

Requirements: Bachelor’s Degree in Psychology, Rehabilitation, Social Work or other related mental health discipline, plus 3 years experience working in a similar setting and/or working with people with mental illnesses. The individual in this position must have good verbal and written communication skills as well as the ability to make sound judgments regarding client care.
