GENERAL PURPOSE OF JOB Support and coordinate routine Community Services operations and documentation requirements for the department; ensure community services are provided in accordance with the person supported needs and with the intent of developing independence and choice within the scope of their skills and abilities; ensure compliance with all Coarc policies and procedures and all applicable regulatory requirements. ESSENTIAL DUTIES AND RESPONSIBILITIES

Facilitate the necessary support services to people receiving Community Services; coordinate community development contacts and community level contacts to ensure that the community services are occurring within the integrated community.

Schedule activities and staff assignments to achieve the expected quality and fiscal outcome for the service.

Provide training and ongoing oversight to assigned staff for routine documentation and computer usage related to required and standardized reporting; oversee quality measures for reporting and documentation to ensure all regulatory standards are met for billing and documentation of services.

Promote interdepartmental cooperation for routine operations and areas of program development related to communication, service delivery, efficiency, and outcomes.

Responsible for teaching staff the requirements for all types of Community Services plans and for training staff in the use of software databases and other electronic programs used to prepare, update and maintain documentation.

Responsible for Writing, distributing, uploading and implementing Staff Action Plans, assessments and other related Community Services coordination.

Responsible for assisting in the development and maintenance of Coarc's Self Advocacy Committee, provide and offer training opportunities to the group, assist the President to Coordinate Meetings, agenda items and outcome achievements.

Coordinate department level services with other Agency program teams and with related outside agencies on behalf of individuals receiving services.

Provide supportive counseling to assigned people supported to promote satisfying outcomes.

Responsible for department level logistics operations, related training and monitoring of efficiencies for all departmental operations; responsible for scheduling transportation if needed.

Assist with departmental and program improvement initiatives as requested by Supervisor or other management staff.

Requirements of the position:

Associate's degree preferred

Minimum of one to two years related experience

Exceptional written, communication, and computer skills

A valid driver's license.

Ability to be certified in all required trainings

Occasionally required to lift up to 50 pounds

Able to work independently
