Gouverneur Hospital is certified for 25 beds and services include inpatient detox, substance use disorder rehabilitation, emergency care, imaging and EKG's, physical therapy, and respiratory therapy.STATUS: Per DiemLOCATION: Gouverneur HospitalDEPARTMENT: Physical TherapySCHEDULE: 8 hour variable shiftATTRIBUTESRequired: graduate of an accredited associate degree program, PTARequired: active and clear NYS PTA licenseBLSRequired: 3 months of experience derived through educational field work required RESPONSIBILITIESAdministers patient treatment plan involving application of physical therapy techniques and modalities as indicated and under the supervision of the physical therapist. Monitors response and progress to update and discusses modifications to the treatment plan with the physical therapist in order to meet patient goals. Provides for audiology screening services.Instructs patients and family members / caregivers in physical therapy procedures and self-care techniques. Directs care that is provided by the physical therapy assistant by providing oversight. Assists with discharge planning and documentation to provide discharge instructions and maintain post-discharge continuity of care. Documents additional information that is not performed during evaluation and treatment sessions.Documents and records effects of treatments, response, and progress. Incorporates quality improvement strategies in clinical practice.Participates in professional education, training and development. Consults with the physical therapist, physician and other health care personnel regarding treatments.Assists in tracking, obtaining insurance authorizations, scripts as needed, scheduling and registering of patients and all other duties as assigned.PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.PAY RANGE: $26.54 - $38.08The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
