They are seeking a compassionate and skilled Social Worker to join our interdisciplinary team, focusing on the needs of the frail elderly population.Job Responsibilities:
Assist participants in understanding and utilizing community health and public services, including SNAP, SC/DRIE, Medicaid, Medicare, and housing assistance.
Coordinate communication and care during/post-hospitalization, ensuring participant preferences are honored and documented.
Conduct timely outreach following hospitalization, providing ongoing support and collaborating with clinical teams on treatment and discharge plans.
Offer social work consultations, including education on treatment options and arranging supportive services such as bereavement assistance and caregiver support.
Complete comprehensive biopsychosocial assessments and facilitate advance directives documentation.
Develop and apply participant-centered care plans focused on psychosocial goals.
Leverage Electronic Medical Records (EMR) for assessments and documentation, ensuring timely completion.
Manage a caseload to address social, financial, and psychological challenges impacting health and well-being.
Collaborate with healthcare professionals, attend interdisciplinary team meetings, and present social work interventions and recommendations.
Facilitate psycho-educational sessions for participants, promoting the role of social work within the team.
Participate in ongoing professional development and represent CenterLight Healthcare at community conferences.
Qualifications:
M.S.W. from an accredited college or university.
Minimum of one (1) year of experience in a healthcare setting, ideally working with frail or elderly populations.
Experience in home care and fieldwork preferred.
Active NY State Social Work license (LMSW or LCSW) required.
Access to a car for field visits is preferred but not mandatory.
Must meet state regulations to practice social work and maintain current immunizations.
