CDPAS CoordinatorFarmingdale, NY 11735, USA ● Jamaica, Queens, NY, USA ● Staten Island, NY, USA Req #11928 Wednesday, April 3, 2024Centers Plan for Healthy Living's goal is to create the ultimate healthcare experience that provides our members, their families, healthcare decision makers, and general caregivers with the guidance and plans they need for healthy living.JOB SUMMARY:The Intake CDPAS Coordinator is responsible to review the CDPAS documents obtained by the Field Intake RN and Intake Social Worker team, ensuring quality and accuracy on documentation and compliance with DOH and CPHL policies.PRIMARY RESPONSIBILITIES:

Assist and assign Intake Social Worker in outreach and education process for prospective members that have expressed interest in the CDPAS program.

Collaborate with MD offices to ensure DOH-4359 (2010) Physician’s Order for Personal Care/Consumer Directed Personal Assistant Services is signed and obtained in a timely fashion to ensure quality and accuracy.

Notify Fiscal Intermediary Service about the prospective member’s interest in CDPAS program as a way to initiate vetting and hiring of Personal Assistance(s).

Facilitate timely communication and completion of all documents by Field Intake RN and Intake Social Worker, so safe plan of care can be determined and implemented.

Notify business development of outcomes for CDPAS referrals.

Monitor for proper documentation in Case Trakker.

Provide Field Intake RN and Intake Social Worker with feedback and education as needed in regards to proper completion of all documentation needed.

Ensure The Consumer Directed Personal Assistance Program Application and Consumer/Designated Representative Acknowledgement of Roles and Responsibilities for Receiving Consumer Directed Personal Assistance Services forms are obtained within thirty days of receiving MD orders, and review to ensure quality and accuracy.

Identify when CDPAS prospects have completed processing, and submit CDPAS authorization to Utilization Management.

Daily tracking, and weekly reports to management on status of all CDPAS cases for upcoming enrollment period.

Communicate with Care Management and Social Work department to guarantee smooth transition when passing unprocessed CDPAS referrals to Care Management.

Assist with obtaining and completing Access NY Supplement A forms as necessary accompanying financial documents required to ensure quality and accuracy in submission to the state.

EDUCATION AND EXPERIENCE:

Education

Required:

Excellent written and verbal communication skills

Proficiency in the use of Microsoft Office tools

Preferred:

Associates or Bachelorette degree

Bilingual preferred - Chinese, Korean, Albanian, Creole

Type of Experience

Required:

Computer proficiency

Preferred:

Knowledge of current community health practices for the frail adult population, behavioral health and cognitively impaired seniors and disabled including knowledge of physical health, aging and loss, appropriate support services in the community.

Understanding of and sensitivity to member’s cognitive ability, means of communication and cultural differences and needs of the community are essential.
