We want to be sure that every day is filled with moments that matter — moments of genuine pleasure, moments of genuine purpose and moments of genuine peace.As a member of our caring, compassionate health care team the RN MDS Specialist/Coordinator will enjoy: Tuition assistanceHourly position – eligible for great shift differentials Multiple units providing long term care, memory care and/or med surg rehabilitation careNon-direct care role, but still get to be around residentsGreat co-workersSupportive work environment and thorough orientationFriendly and welcoming work atmosphereEasy commute from Troy, East Greenbush, Colonie and Latham Shadow experiences for those interested in getting additional insight on this rewarding opportunityThe RN MDS Coordinator will be responsible for, but not limited to:Oversee all aspects of Medicare requirements and MFD coordinationResponsible for ensuring that the MDS is submitted timely and accurately reflects the services providedWork with others to coordinate communication among all clinical staff Coordinates Medicare PDPM, PPS and OBRA care plan schedules for the facilityWork collaboratively with facility MDS RN Specialist and SPHP MDS TeamResponsible for ensuring completion of required documentation for Medicare residents (admission through discharge)Monitor MDS data and other documentation for accuracyParticipate in determining Medicare Part A eligibility and other MDS considerationsAct as a MDS/PPS resource to the interdisciplinary teamPerforms electronic submission of the facility-wide MDS information timely to the state of New YorkActively participates in resident care planning providing CAAs reports to the nurse managersEnters and maintains residents' ICD-10 codes in database, coordinating with medical records for updating face sheetsMinimum Requirements:Valid NYS RN licenseAssociate’s degree in nursingCurrent CPR and BLS or achieve with onsite trainingMDS Certification or must achieve within designated time period with the site's assistance in obtainingPreferred Requirements:Bachelor’s degree in nursingRAC-CT Certification preferred2+ years of experience as a RN in a skilled nursing facility or 5 + years in a direct care role (LPN, C.N.A)3+ years of experience as a MDS coordinatorPay Range:$34.50 - $47.95 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
