The Care Navigator is responsible for coordinating and supporting Mental Health care within the clinic and for coordinating referrals to clinically indicated services within and outside of the clinic. The Care Navigator may provide evidence-based treatments to patients or collaborate with other Mental Health providers when such treatment is indicated. This role will report to the Manager, Behavioral Health/Primary Care Integration or designeeSTATUS: Full Time (40 hours)LOCATION: Clifton Springs HospitalDEPARTMENT: Inpatient Chemical DependencySCHEDULE: Day, evening, and night rotationATTRIBUTESMinimum of 2 years of experience in Human Services related field, medical related field, client/patient interfacing experience required.RESPONSIBILITIESSupport and coordinate behavioral health care with the patient, patient's Primary Care Provider (PCP), BHC, psychiatric provider and other Integrated Care team members. Provide patient education about common mental health and substance abuse disorders and available treatment options.Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.Provide or facilitate referrals to mental health clinic/substance use disorder treatment programs as clinically indicated.Participate in regularly scheduled Integrated Care team meetings and communicate treatment recommendations to team members and outside collaborators as indicated.Facilitate patient engagement and follow-up in care.Handle confidential information with discernment and sensitivity.EDUCATION: AS (Required)PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.PAY RANGE: $19.25 - $25.06The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future.
