NEW RN GRADUATES ARE WELCOME
Schedule: 12/hr and 10hrs shifts available; days, nights, weekends
RN Pay Range: $37 - $45/hr Depending on experience; night differential available as well
Job Duties Include:
Provide holistic care to the individuals, their family members and communities/population, per plan of care
Complete assessments (Initial and ongoing) and care plans
Create education and individualized care plans
Collaborate with the interdisciplinary health care team and other providers as needed
Develop, coordinate and update individualized plan of care based on evidence in collaboration with the indivdual's family, interdisciplinary team, and medical home providers to promote continuity of care acros the continuum.
Accurate, thorough and timely documentation
Attend and Participate in meetings and conferences
Requirements:
Associates Degree in Nursing or higher
Current NYS RN License and Registration
OPWDD experience is a plus
2+ years of Nursing experience preferred
Current Basic Life Support Certification
Experience working with individuals with intellectual or physical disabilities a plus
Experience with SigmaCare and/or E-Clinical Works a plus
