OverviewElderwood Independent Practice Association (IPA) is seeking an Outreach Specialist to join our incredible growing team of professionals. Why should you join Elderwood IPA?

Monday-Friday Schedule w/ no weekends or overnights

Hybrid working options

Medical, Dental & Vision upon 1st of month following 60 days of hire

Up to 140 hours (19 days) of PTO time PLUS six (6) holidays within the 1st year

401(k) retirement plan with vested employer match up to 4%

Substantial Referral program

Responsibilities

Answer product and/or service questions.

Travel to and provide information to community agencies to help facilitate referrals

Complete all necessary paperwork, documentation, and follow-up as required (i.e. lead reports, outreach logs, scheduling and other administrative work).

Establish contact, assess individual's needs and concerns, provide overview of organizational mission, effectively and clearly present benefits of products in accordance with all Governmental regulations.

Establish and maintain key relationships within designated territory to generate potential enrollment opportunities.

Participate in community events in collaboration with the Director of Business Development

Assist with the screening of candidates toward identification of qualified, eligible potentials related to both long-term care needs and Medicaid coverage for enrollment

Provide potential members and their families with education regarding the MLTC model of care

Help process calls from interested parties for potential enrollees

Complete pre-enrollment screening form for all potential enrollees

Assist Enrollment Coordinator with enrollment responsibilities

QualificationsLicensure: Valid NYS Driver’s License Education: Associates degree required, Bachelors preferred Bilingual fluency in Spanish, highly preferred Additional Requirements:

Must be in good standing with the Medicare and Medicaid programs.

Proficiency in Microsoft Office: Excel, Word, Access; Internet navigation, required

Ability to work a flexible schedule, including nights and weekends, required

Ability and willingness to travel frequently within an assigned geographic region, required

Presentation Skills

Excellent Communication

Ability to solve diverse problems that require interpretation of administrative and/or technical issues.
