Under the general direction of the FTC supervisor, the FTC facilitator will facilitate a variety of family team conferences (FTCs) within the department. The FTC facilitator will review all cases in Connections in preparation for the conference. The FTC facilitator will assess and evaluate the family for risk / safety issues during reviews. When necessary, the FTC facilitator will participate in team meetings prior to upcoming FTCs. The goals of the conferences are to identify risk issues, prevent abuse and neglect; keep families together; stabilize and maintain family functioning. The FTC facilitator will work 35 hours per week, 9 – 5PM and evenings when necessary. Conferences will occur within the family’s home or the office. Responsibilities

Facilitate family team conferences by utilizing consensus decision making process to ensure involving children, families, service providers and other supports.

Ensure all safety and risk concerns are identified and addressed in conferences.

Make recommendations to case planners and other service providers, including referrals to potential resources.

Collaborate and strengthen partnerships with community representatives to ensure participation and involvement in conferences.

Complete administrative responsibilities, including the timely progress note entry of family team conferences in CONNECTIONS and filing FTC template in the case record.

Facilitate at minimum two to three conferences per day. Attend inter/intra agency meetings to share information, develop policies and procedures.

Maintain a tracking system of cases facilitated and outcomes.

Other related duties as assigned by supervisor.

Qualifications

Bachelor’s degree in social work or related field.

Three years’ experience practicing social work utilizing casework, group work or community organization methodologies.

Knowledge and skill with group conferencing processes.

Ability to lead diverse and multicultural groups toward resolution.

Good organizational, written and verbal communication skills.

Ability to develop working relationships with children and adults using good interpersonal skills.

Ability to accommodate a flexible work schedule, including evenings.

Exceptional mediation skills to effectively facilitate group conference situations.

Ability to summarize clearly a variety of competing options and to facilitate consensus building.

Bilingual (English/Spanish) a plus
