Position SummaryProvides day-to day operational leadership, oversight and support to the Acute Case Manager and Utilization Review roles. Leads, and facilitates in utilizing current practice and process to achieve optimal patient, clinical, and operational outcomes in timely, coordinated, and cost- effective manner. The Supervisor- Acute Case Manager supports the Acute Case Manager team in using industry accepted, outcome-based approaches. The Supervisor - Acute Case Manager also maintains a identified Case Manager assignment and supports in assessing, planning, and facilitating individualized continuum of care plan for patients, based on assessed needs and available resources for the Acute Case Manager team. The Supervisor- Acute Case Manager collaborates cooperatively with members of the interdisciplinary health care team, the patient, community health care providers and third- party payers to assist in ensuring that the patient receives the needed services in a timely manner in the most cost-effective setting. The Supervisor -Acute Case Manager leads with taking proactive approaches in ensuring the integration of clinical and operational outcomes through analysis of clinical and financial data, including length of stay and DRG profiling.ExperienceAn R.N with five minimum years clinical experience in an acute care setting; a minimum of three years, Acute Case Manager experience; demonstrated leadership and autonomy in nursing practice; demonstrated capacity to lead projects and teams ; Demonstrated effective verbal and written communication skills; knowledge and experience with payorsEducationBSN Bachelors Degree RN: A BSN degree or a BA in addition to their R.N. degree. A R.N. with 5 years experience willing to pursue and complete a BSN/BA may be considered. Individual consideration may be given for a person with a licensed degree in a related field with significant relevant clinical background.
