In addition, the Family Support Services Manager will provide supervision and training to a team of Community Habilitation and Respite providers. Specific duties and responsibilities include, but are not limited to:

Develop Community Habilitation Plans and/or Respite Care Plans that are reflective of the persons needs, valued outcomes, and goals

Facilitate and document meetings with the people supported, family, service providers, involved agencies, and necessary professionals to review Community Habilitation plans, significant events, incidents, and/or behavioral concerns as necessary

Conduct home visits with enrolled families/individuals, as needed

Document initial reviews, monthly progress, and semiannual reviews to ensure measurable and qualitative progress in the areas of independence, inclusion, individualization, and productivity

Responsible for the supervision of assigned staff, including interviewing, onboarding, and completion of evaluations within the allowed timeframe

Review & approve time sheets for Community Habilitation providers, and maintain oversight of units of service to ensure efficient utilization

Oversite and completion of billing and all required elements per regulatory standards

Complete DDP1’s, DDP2’s, DDP4’s, and assessments relevant to an individual receiving HCBS Waiver Services

Responsible for addressing and reporting all incidents, accidents, observed or suspected situations, which adversely impact on the health and safety of any/all individuals receiving services

Responsible for complying with all Part 635, Part 671, Part 624, and Part 633 regulations in total, and reporting violations, deficiencies, and/or incidents to direct supervisor as they are discovered or indicated

Facilitate training of providers to meet the needs of people served by programs

Monitor and manage the use of Community Habilitation and In Home Respite units by conducting intakes as needed and assisting in the recruitment and retention of Direct Support Professionals

SPECIAL REQUIREMENTS:

Possess excellent interpersonal, written, oral, and communication abilities and effective time management/organizational skills

Work scheduled hours that may include mandatory overtime, weekend and/or holiday timeframes. Also maintain an acceptable work attendance as defined in the agency’s employee policy manual

Be computer literate with knowledge in agency operating systems and applicable software required for department needs

Able to generate and present comprehensive written reports and respond to questions about the report

Demonstrate sound judgment in normal work and emergency situations

Must physically be able to perform required certifications

Bachelor’s Degree in Human Service or related field that meets or exceeds the requirements for QIDP certification and one year of full time experience working with people with differing abilities

A valid drivers license with record acceptable to the agency's insurance carrier
