Summary of Position: Case Manager (SOR) The Case Manager (Full-time) is responsible for provision of case management services and crisis intervention for Outreach Bellport Women's Outpatient program. Job Specific Criteria:

Case management and crisis intervention to assist in the coordination of treatment services.

Advocates on behalf of clients to obtain needed services.

Interfacing with appropriate agencies and systems (DSS, probation, courts, etc.) on behalf of the client and assisting clients in meeting all mandates.

Ongoing case management related to housing, transportation, childcare, food and other needs to support the client’s recovery efforts .

Conducts toxicology tests, as needed.

Attend and participate in staff/community meeting where appropriate.

Provide crisis intervention, assess client needs and facilitate group services and back up coverage, as needed.

Provide support to and educate clients in relapse prevention and overdose risk awareness

These goals are intended to provide each job holder the knowledge they need about expected performance. Accomplishments against these goals will be a discussion point in the annual evaluation.

For this Long Island, NY position, the expected base salary is $38,000.00 - $50,000.00 / year.

Standard Criteria:

An individual who is a provisional qualified health professional or qualified health professional such as a CASAC, LMHC, LMSW, LMFT, or limited permit holders of similar credentialing (MHC, MSW, CASAC-T, etc).

Interacting with staff, clients, students and visitors in a courteous, professional and respectful manner that is consistent with the agency’s mission and values.

Producing accurate, neat and thorough work.

Prioritizing and planning work time efficiently; effectively managing competing demands and dealing with changes, delays and unexpected events.

Understanding and consistently complies with agency policy and procedures (including time and attendance, dress code, etc…).

Supporting a positive work atmosphere by behaving and communicating in a manner that allows the employee to get along with clients, students, coworkers and supervisors.

Communicating (in writing as well as orally) clearly and concisely

Illustrating flexibility and adaptability in role and function when circumstances warrant; (demonstrates openness to assist in needed responsibilities in evolving situations).

Maintaining appropriate boundaries, ethics and professionalism; complying with agency, OASAS and other professional code(s) of ethics; seeking supervision when challenges arise.

Ensure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan and personnel policy and procedure.

This job operates in a professional office environment and outside in the field. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. Must be able to travel among assigned sites and locations as needed.
