Job Requisition: REQ2812 Employment Type: Full time Shift: Rotating Hours Per Week: 40 hrsLocation: Cancer Medicine Center and Inpatient PharmacyHiring Range : $53.00- $73.40 The actual compensation for this position will be determined based on experience and other factors permitted by law. Responsibilities: Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems. Recommends formulary alternatives when appropriate. Compounds and dispenses medications for accurate delivery to patients. Provides drug information and education to medical staff and patients. Communicates with physicians and other clinicians as necessary to facilitate care.
Oversees technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. Assists with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs.
Provides patient counseling efficiently and effectively when required.Minimum Qualifications Licensure / Certification / Registration: Required Credential(s): Pharmacist credentialed from the New York Board of Pharmacy obtained prior to hire date or job transfer date.
Must be able to speak, read, write and follow instructions in English.
Education:
Bachelor's degree in Pharmacy or Doctorate of Pharmacy required. Pharmacy residency preferred.
Complexity of Work: Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment.Personal Protective Equipment: Follows Standard Precautions using personal protective equipment as required.
