Our goal is to offer affordable, accessible, and holistic-centered care that empowers individuals to reach their full potential and enhance their quality of life.Job Summary:You will serve as a member of a team that responds to individuals experiencing a non-life-threatening mental health and/or substance use disorder crisis when dispatched through the 988 Suicide Crisis Lifeline Call Center to adults in the Bergen and Passaic County communities on a 24-hour basis, 365 days a year. Respond to adults in Bergen and/or Passaic County upon dispatch by the 988 Suicide and Crisis Lifeline Centers, providing timely crisis intervention and crisis-de-escalation services for non-life-threatening mental health and/or SUD crises

Respond and coordinate care with a Crisis Response Worker and provide support, linkage to ongoing community mental health and/or SUD services, and initiate follow-up contacts to ensure further crisis diversion

Provide transportation to crisis stabilization and receiving centers, hospital emergency rooms, or CCBHCs when safe to do so and the individual is voluntary for these interventions

Refer individuals to services that will help mitigate further risk of future crises by assessing/addressing their immediate needs

Collect and documents consumer's progress in assigned databases, as per DMHAS,with all entries authenticated and dated.

Work with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data

Conduct follow-up visits when appropriate to ensure consumers receive required level of care; research and coordinate resources for service provision and provides psychoeducation

Collaborate and coordinate care with available family members, friends/ an/or loved ones to support the individual in crisis

Recognize and address gaps in service and actively collaborate for resolution

What We Need to See:

Lived experience with 2 years of continuous recovery from a serious mental illness or substance use disorder.

At least 2 years of field experience working in the community with consumers.

Certified by at least one of the following credentialing entities: Addiction Professional Certification Board, Mental Health Association in New Jersey, Association for Addiction Professionals, Certified Wellness Coach, Community Mental Health Associate.

A valid driver's license and must be insurable under Agency's automobile insurable policy.
