This is a challenging and rewarding position that requires someone committed to assisting low income individuals/families infected and affected by HIV Disease. JOB OVERVIEW:
Biopsychosocial assessments, service coordination plans, treatment adherence, patient education on HIV/medications/symptom management, case coordination with medical providers, support with medical/insurance benefit applications. CAPCO is not-for-profit social service agency so there is no risk for exposure to body fluids!
QUALIFICATION/REQUIREMENTS:
Bachelor’s Degree with experience preferred.
MPH, MSW, LPN, RN, or other medical professional will also be considered.
Bilingual preferred Spanish/English (speak, read, write)
Health Education, Case Management and/or HIV experience a plus.

Highly Motivated, Independent, Professional

Compensation/Benefits
Full-time (40 Hour work week) Salary contingent upon qualifications, experience and credentials Health, Dental, and Life Insurance 401K with up to 4% matching Paid Leave: 10 Vacation Days, 12 Sick Days, and 4 Personal Days. 12 Paid Holidays No regular weekends
