Job Description:
We are seeking a compassionate and experienced Registered Nurse (RN) to join our home care agency. As an RN in our organization, you will be responsible for providing comprehensive nursing care to patients in the comfort of their own homes. Your primary goal will be to promote patient health and well-being while assisting them in achieving maximum independence.
Primary Responsibilities:
Conduct initial assessments of patients' health status and develop individualized care plans.
Work with home health aides implement care plans and periodic follow-up evaluations.
Home safety assessment according to patient needs.
Adhere to all regulatory requirements, professional standards, and agency policies and procedures to ensure quality care delivery and patient safety.
Qualifications:
1. Valid New Jersey RN license
2. Minimum of one year of nursing experience, preferably in home care or a similar setting.
3. Excellent clinical assessment and critical thinking skills.
4. Strong communication and interpersonal abilities.
5. Reliable transportation and willingness to travel to patients' homes within the assigned geographic area.
