Don’t miss this opportunity to join a rapidly growing and well-regarded agency and help families throughout NH.The Roadmap to Reunification Clinician (R2R) serves as a facilitator to support connections and seamless care for children and families attempting to navigate various systems throughout the state of NH. Service coordination will be focused on bringing together members of the clinical team, NH DCYF, birth parents, caregivers, and all collateral services and providers related treatment services and program documentation.Responsibilities:

Serve as a liaison and facilitator for the clinical team, the client, the family, and all collateral service providers throughout NH

Present to in-person meetings to support care coordination.

Document per New Hampshire state regulations

Meet with the treatment team and caregivers consistently to ensure proper implementation of the treatment plan, address specific target areas, review

overall progress toward goals, and discuss changes to the treatment plan if needed

Ensure compliance, accuracy and quality of documentation and clinical records

Ensure compliance with Federal, State and agency guidelines related to administrative and clinical policies

Ensure clients receive quality and professional services

Respond to parent/caregiver/family concerns and determine appropriate resolutions

Participate in Clinical Team Meeting and other meetings as assigned

This is a Full Time, 40-hour per week position.

Qualifications:

Masters Degree in Counseling, Social Work, Psychology or related field.

At least 1 year of experience working with children and adolescents identifying barriers and resources to reunification with families

Demonstrated knowledge of the needs of children and families in the child welfare system

Valid driver’s license with reliable and consistent access to a vehicle
