OverviewAs delegated by a registered nurse, performs patient care activities including constant, close observation of an assigned patient and assistance with activities of daily living. Maintains a safe and clean environment in accordance with D-H policy and procedure.Responsibilities
Reports to RN at beginning of shift for communication about assignment and delegation of tasks related to care and observation of the assigned patient. Completes any related required learning activities as directed by the RN.
Remains in constant attendance with the assigned patient until relieved by a qualified individual identified by the RN.
Assists in meal tray set-up and may assist with feeding if necessary and appropriate as directed by RN.
Offers assistance with hand or face washing as needed.
Assists nursing staff to turn or reposition patient as directed.
Monitors and records intake and output as directed by RN.
May ambulate/toilet patient if appropriate and directed by RN.
May assist with bathing the patient as directed by RN.
Reports any observed changes in the patient's condition to the RN.
Obtains immediate assistance from the RN in the case of any unusual circumstances, or with any restrained patient who needs assistance or attention.
Does not convey, handle, administer or dispose of medications and does not have access to secured medication areas.
Performs other duties as required or assigned.
Qualifications
High School Diploma or GED required.
Ability to relate easily to both children and adults with all types of illnesses.
