Our Mobile Crisis Response Team (MCRT) specifically offers community-based supports to individuals in need wherever they are albeit at home, work, or anywhere else in the community where the person is experiencing a mental health crisis. Our services are specifically aimed at diverting individuals from emergency room care, unnecessary hospitalizations, and incarceration by collaborating with first responders, health care providers and community-based agencies to stabilize a psychiatric crisis and initiate immediate treatment. The Acute Care MCRT Clinician provides assessment, triage, and referral to appropriate services. This is a full time, 38 hour per week position. Schedule available: 2 days and 1 overnight during the work week, 12-hour shifts plus 2 hours of supervision weekly.Learn more about Acute Care Services and Mobile Crisis Response: https://youtu.be/hA1rnlKBjQc Major Functional Areas and Description of Duties

Answer incoming calls as needed to triage general behavioral health needs and crises;

Provide thorough psychosocial and diagnostic assessment to clients in emergent situations to determine best available treatment options to reduce and/or ameliorate psychiatric crisis;

Assess the need for psychiatric hospital admission and if needed, facilitate admission and obtain insurance authorization;

Provide differential diagnostic treatment, short-term crisis stabilization and counseling support to further mitigate risk and promote wellness;

Provide timely documentation on each service delivered;

Work collaboratively with other treatment team members, natural supports, hospital-based staff, and other external providers to ensure wrap-around care is provided to the client;

Serve on appropriate committees, both internal and external, as determined by supervisor;

Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;

Execute all required services and documentation in accordance with agency policies and federal/state standards;

Maintain professional memberships, licenses, accreditation, certifications, etc.;

Represent CLM in a professional and appropriate manner in all settings;

Perform all other duties as assigned by supervisor.

Qualifications and Skills

Master’s degree required;

Licensed or license eligible (LICSW, LCMHC or LMFT) in the State of New Hampshire, preferred;

Minimum of two years of direct mental health experience;

Crisis intervention experience, training, and/or education, preferred;

Experience working with co-occurring disorders preferred;

Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations;

Proficient computer skills, specifically with all Microsoft office applications;

Ability to document effectively and efficiently in course of clinical work;

Ability to navigate an electronic health record;

Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;

Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;

Demonstrate effective communication skills, organizational skills, and ability to multi-task.

Physical Demands and Working Conditions

The work takes place in various community settings, within the client’s home, and in the office. May need to transport clients at times when deemed safe to do so. Manual dexterity is needed to use computers and standard office equipment. The ability to sit or stand for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.
