Care Services Coordinator

This New Hampshire based Care Services Coordinator will have a direct impact on the lives of individuals living with ALS. The ideal candidate for this role is a health and human services professional (ie: a social worker, case manager, nurse, or allied health professional). Experience related to ALS symptoms, progression, and care issues is preferred, since this role requires regular engagement with those diagnosed with ALS and their caregivers. This role requires an individual who will respect the individual autonomy of patients, while delivering and coordinating support services with sensitivity, to preserve the dignity and respect of each individual. ESSENTIAL FUNCTIONS: Performs, in collaboration with the Care Services Manager, complex care management tasks.

Act as a patient advocate across the continuum of care, to facilitate and promote quality of life, and minimize fragmentation of health care delivery; for approximately 75-100 clients.

Guide families living with ALS through the resource available for them, and consult on decision making regarding care.

Must be able to regularly travel throughout the assigned territory.

Conduct home assessment visits, in-person and virtual,

Deliver and picking up durable medical equipment (must be able to lift to 30lbs).

Develop care plans, outreach strategy, and enrolling new clients.

Work collaboratively with team members at ALS Clinics and ALSA Certified CentersSM to ensure effective communication and continuity of care for patient services.

Perform patient assessments, advocacy, clinic visits, clinical coordination, family education and conferences, and other related areas.

Identify the short and long-term needs for chronically ill patients and their families, using established programs, policies, procedures, and guidelines.

Record case notes in a timely manner.

Assess client needs.

Develop care plans, and advocate for the coordination of social/health services.

Plans and facilitates monthly support group(s) for clients and families.

Provide educational programs, and act as a resource to the community and other staff.

Manage the resources at your disposal.

Provides in-service education programs to a local hospice, home health agencies, hospitals, skilled care facilities, and other state social service agencies.

Administrative functions: Ensure that all materials provided and utilized are in accordance with ALS Association policy and best practices; maintains confidential document and computer-based files of clients’ information and retrieval system for interrelated files and records; maintain; prepare regular reports of activities; prooing, detail-oriented and ability to format letters, memoranda, and reports using Microsoft Office programs.

Polished professional demeanor, excellent interpersonal skills in dealing with all staff, Board members, organization clients, private corporations, and/or public offices; excellent organizational and time management skills.

OTHER RESPONSIBILITIES:

This position requires travel within a designated area. May require travel outside of New Hampshire for regional or national meetings. This position will require the use of an employee’s automobile, which should be dependable and have adequate insurance coverage. Working hours may include evenings and weekends, outside regular office hours.

QUALIFICATIONS:

Bachelor’s degree or Masters degree in public health, social work, or related area.

A minimum of two years of experience in health care or community service/relations setting, which include assessments, care plan, ongoing advocacy, with a history of high-level of performance.

Knowledge of clinical and ethical regulations.

Proficiency with Microsoft Office suite and an understanding of databases.

Attention to detail, highly efficient, and conscientious about thorough follow-up.

Strong critical thinking, and problem-solving skills.

Effective, professional verbal and written communicator.

Team player who enjoys interaction with professionals, peers, volunteers, and those served by the organization.

Must have and maintain a valid drivers’ license.

Ability to sit for long periods.

Ability to move about to retrieve files and receive clients.

Ability to transport and deliver light medical equipment (i.e., portable wheelchairs).

Remote when not required to travel. A candidate in central New Hampshire is preferred.
