Position Overview:
We are seeking a compassionate and experienced Home Health Care Nurse to join our team. The Home Health Care Nurse will be responsible for providing skilled nursing care to patients in their homes, coordinating care plans, and collaborating with other healthcare professionals to ensure the best possible outcomes for our patients.
Responsibilities:
Conduct comprehensive assessments of patients' health status and develop individualized care plans.
Administer medications, treatments, and procedures according to physician orders and established protocols.
Monitor patients' progress and condition, and communicate any changes to the healthcare team.
Provide education and support to patients and their families on managing their health conditions and promoting wellness.
Coordinate care with other healthcare professionals, including physicians, therapists, and social workers.
Document patient care and progress accurately and timely in electronic medical records.
Follow all applicable laws, regulations, and agency policies and procedures.
Participate in ongoing training and professional development activities.
Qualifications:
Current RN license in NEVADA.
Minimum of 2 years of experience in nursing, with experience in home health care preferred.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal abilities.
Ability to work independently and collaboratively within a multidisciplinary team.
Proficiency in using electronic medical records systems.
Valid driver's license and reliable transportation.
CPR certification (BLS for Healthcare Providers).
