We are an equal opportunity employer including veterans and people with disabilities.A Brief OverviewFacilitates the QR conversion of Project Austin patients by creating a new patient outreach encounter from the Electronic Medical Record (EMR) to the AdaptCare application. Monitors, maintains, and verifies all required patient information within the EMR and AdaptCare application to ensure consistency and accuracy. Works with Project Austin clinical team to ensure a team-based approach of the QR conversion to provide a safe transition of the electronic care plan. Promotes a welcoming and reassuring positive interaction in each patient and family encounter. Must have a strong skill set in technology and be able to multi-task.Essential Functions

Converts patients QR code with complete and accurate information on the plan of care according to Parent QR Conversion Tip Sheet.

Creates new patient outreach encounters from the EMR to the AdaptCare application to convert files.

Provides, collects, and accurately documents all required patient information with each new patient outreach encounter.

Utilizes Children’s Connect to send caregivers instructions on how to download the AdaptCare application.

Contacts patients/families to confirm patient information, verify demographic and the AdaptCare application is downloaded successfully.

Documents patient conversions and signs encounters

Answers patient/family questions about AdaptCare application and refers to Project Austin clinical team as appropriate.

Education Qualifications

High School Diploma Required

Experience Qualifications

1 year Customer service experience Preferred and

1 year Experience in health care setting Preferred

Skills and Abilities

Excellent verbal and written communication skills, including proficiencies in digital communications. (High proficiency)

Strong computer proficiency, including keyboarding skills and working knowledge of Microsoft Office products. (Medium proficiency)

Reads routine sentences, instructions, regulations, procedures, or work orders; writes routine sentences and completes routine job forms and incident reports, speaks routine sentences using proper grammar. (High proficiency)

Office: This job primarily operates in a professional office environment and routinely requires the use of standard office equipment such as computers, phones, copy machines, etc. (Medium proficiency)

Knowledge of medical terminology preferred. (High proficiency)

Licenses and Certifications

BLS - Basic Life Support Preferred
