Reviews patient charts, consults with the appropriate personnel and gathers information as necessary to properly evaluate the patient’s needs before or during the first patient contact, as evidenced by documentation of thorough and accurate patient history on initial evaluation.Ensures insurance authorization is obtained prior to initiating the evaluation.
· Includes thorough PMHx, social and functional history.
· Clearly identifies patient problems.
· Performs appropriate reproducible measurements and tests.
· Includes thorough assessment reflecting the consequences of disease/pathology at the level of the body/person/society.
· Clearly identifies a plan to be followed, taking into account a patient’s diagnosis order and makes appropriate recommendations for the next level of care.
· Includes as appropriate neuromuscular, musculoskeletal, cardiopulmonary and integumentary systems.

PA Physical Therapist License required.Master's/Doctorate degree in Physical Therapy required.One year of general experience within the field and additional training with emphasis in acute care/rehab.One-year experience in Home Care preferred. A valid driver's license with vehicle registration and proof of current insurance on a vehicle used in the performance of the job.
