Phoenix Home Health & Hospice is seeking an Intake Specialist for Comprehensive Disease Management department to joining out Wichita, Ks team!Come Soar with team Phoenix!Our CDM program is committed to connecting with people who would benefit from assistance in navigating the healthcare system. Phoenix's CDM program is designed to care for people who are living with chronic or life limiting illness and to provide guidance in symptom management and advanced care planning. The CDM Intake Specialist process the referrals that help bridge the gaps in healthcare by identifying the needs of people and communicating with patient's healthcare providers. Often times people make critical healthcare decisions in crisis and Phoenix's CDM teams are able to walk this journey with people so they can make informed decisions that align with their personal goals.Responsibilities
Connecting with patients and establishing consistent face to face encounters.
Managing CDM referrals and making appropriate entries for homecare and community services.
Accurate and professional documentation.
Building and maintaining strong partnerships with community healthcare partners/physicians
Connecting and communicating at a high level with Phoenix colleagues to ensure continuity and collaboration of care
Other duties as assigned
Requirements
Medical Office experience a plus!
Computer proficient
Familiar with Microsoft Office, Word and Excel
Experience with multi line phones
Multitasking in a busy office environment
Hospice or Home Health experience preferred
