The Licensed Clinical Social Worker collaborates with themulti-disciplinary team to provide input in the development of the plan of carefor those patients/clients requiring social work intervention. TheLicensed Clinical Social Worker provides counseling and crisis intervention topatients/clients who have social or emotional problems in a variety of settingsacross the age span. Essential Duties and Responsibilities:
Provides necessary social work intervention
Counsels individuals and families in crisis and help with issues such as poverty, loss and illness
Assesses patient's psychosocial needs, plans for, evaluates and modifies interventions to meet the established patient/client goals
Collaborates with all disciplines to plan and evaluate team goals for each patient/client
Facilitates patient/client and family referrals to community agencies when appropriate
Provides financial counseling when necessary to achieve the treatment goals established by the multi- disciplinary team
May be required to respond to crisis situations, including domestic violence and child abuse
May be required to provide witness services and evidence in court cases
Visits schools to assess academic concerns
Works with government agencies to help clients apply for Medicare and Medicaid when appropriate
Arranges home visits to evaluate a patient/client health, home environment and family interaction/support
Assists patient/family/caregivers in planning for present and future psychosocial needs
Observes and reports patient progress and/or changes in patient status to physician and appropriate multi- disciplinary team members
Adheres to facility and Amergis policies and procedures at all times
