As a Hospice LPN at Elara Caring, You Will:

Align with Elara Caring’s Vision – Ensure all activities support the vision of Elara Caring’s board of directors, executive team, and Home Health leadership.

Promote Excellence in Care – Uphold Elara Caring’s philosophy, mission, and policies to deliver high-quality, patient-centered care.

Maintain Privacy and Confidentiality – Safeguard patient and staff information in compliance with HIPAA regulations.

Deliver Skilled Nursing Care – Provide patient care based on the Plan of Care, physician’s orders, and RN delegation, adhering to established nursing standards.

Integrate Care Goals – Incorporate patient care goals into each visit, documenting progress on skilled nursing notes.

Communicate Significant Findings – Report changes in patient condition, unsafe environments, or other concerns to the RN Case Manager, physician, or clinical supervisor.

Ensure Patient Safety – Implement safety plans utilizing patient, family, and community resources.

Promote Patient Independence – Educate patients on self-care techniques to encourage independence while maintaining high standards of care.

Recommend Care Plan Revisions – Suggest updates to the Plan of Care based on patient needs and collaborate with the RN Case Manager.

Perform Specialized Nursing Tasks – Provide advanced nursing services under the supervision of a Registered Nurse or physician.

Collaborate and Communicate – Work closely with the RN Case Manager and other healthcare professionals for guidance and coordination of care.

Manage Medications Effectively – Document and update medication profiles according to protocol.

Support Patient Well-being – Treat patients and their caregivers with compassion and respect, fostering a positive care experience.

Prevent Avoidable Hospital Visits – Utilize preventive measures to minimize unnecessary hospitalizations and ER visits.

Communicate Positively – Maintain supportive and professional communication with physicians, patients, families, and team members.

Respect Patient Property – Handle patient and family belongings with care and consideration.

Contribute to Team Success – Perform additional duties and participate in special projects as needed.

What You Need to Succeed:

Current LVN License – As required by state regulations.

Experience – At least 1 year in an acute care or equivalent setting.

CPR Certification – With the American Heart Association or American Red Cross.

Compassionate, Positive Attitude – A genuine dedication to delivering quality patient care.

Excellent Communication Skills – To effectively interact with patients, families, and team members.

Reliable Transportation – To visit patients within your assigned area.
