About Department: The Transplant Specialty RN is a member of the Multi-Organ Multidisciplinary team, functioning as a practitioner, patient/family educator, consultant/resource to coordinate the pre-transplant phases of care for the kidney, living donor, and liver transplant recipient candidates within an outpatient setting. About Royal Oak HospitalRecognized by U.S. News & World Report as the #2 hospital in Michigan and one of the top hospitals in the country. It is a major academic and referral center with Level I adult and Level II pediatric trauma designations. The advanced capabilities and clinical expertise within this center have earned it Magnet designation four consecutive times and has the distinct honor of housing Michigan’s first Diagnostic Imaging Center of Excellence.Scope of workUnder the general supervision of a nurse manager or other site leadership with indirect nursing leader support, performs diversified clinical duties by providing nursing care to acutely or chronically ill patients in an ambulatory setting. Coordinates care for the potential kidney & liver transplant recipient and live donor including:

Coordinates care for the potential transplant candidate throughout referral, evaluation and testing

Collaborates with the transplant team to review test results and develop plan of care.

Adds the patient to the OPTN/UNOS transplant waiting list and manages MELD updates as required.

Submits narratives for MELD exceptions for liver transplant candidates on the waitlist as indicated.

Provides education about the transplant process to potential candidates.

Develops and maintains educational/informational materials for transplant candidates and their significant others.

Assists with and participates in Patient/Family Educational Seminars

Manages the hepatology care for transplant patients in evaluation and waitlist phases of care (this may include scheduling testing and procedures)

Prepares agenda and minutes for and facilitates the weekly selection committee meetings.

Attends weekly Multi-Organ Transplant Grand Rounds

Completes Federally required TIEDI forms electronically.

Updates transplant policies and guidelines as needed.

Schedules and conducts in-services for referring physicians’ staff and dialysis units’ staff.

Maintains knowledge of OPTN/UNOS, CMS and transplant policies and procedures in order to ensure compliance with regulatory requirements.

Utilizes EPIC and transplant specific EMR for documentation/communication.

May take organ offer calls, coordinate travel for the organ procurement team, and arrange for potential transplant recipients to be admitted to the hospital

Nursing Procedures may include urinary catheterization, vaccine administration, and phlebotomy if needed.

Participates in department committees (e.g. QAPI, Clinic Operations, staff meetings, etc.).

Participates in QA Projects/activities.

Qualifications

Required Bachelor's Degree In nursing from an NLN-approved school of nursing

3 years of relevant experience Acute nursing experience Required

Two (2) years nursing experience in all or any of the following: transplant, nephrology and hepatology. Two (2) years outpatient/physician office experience (preferred, not required).

Registered Nurse (RN) - State of Michigan Upon Hire required

At least one Certification from preferred list - Upon Hire required

Basic Life Support (BLS) - AHA American Heart Association preferred Or

Basic Life Support (BLS) - American Red Cross preferred

Specialty Certification - CCTC required, or commitment to take certification exam within two (2) years and maintain certification via continuing education.
