Job Overview:

Elmwood Acres Adult Foster Care (AFC) is hiring a House Manager for an AFC working with adults that have mental and/or developmental disabilities. Elmwood Acres stives to offer its residents care that rooted in dignity and respect. The home is licensed to care for 6 residents.

This position operates in a licensed adult foster care home setting. This position while based in a health care setting also routinely uses standard office equipment such as computers, phones, photocopies, filing cabinets, and fax machines.

Duties:

Provide management of the day-to-day activities in the home including:

Uphold operational policies of the home including medication guidelines and ensure residents' rights are known and respected and that the home remains in compliance with State of Michigan, and Home and Community Based Services Final Rule.

Create a structured routine and provide guidance to promote positive behavior and emotional well-being

Provide daily assistance with activities, and care of residents including personal care and hygiene, household support, independent living skills, and activities outlined in the resident's individual treatment plan

Complete resident/client intake assessments and maintain accurate records and documentation for each resident, including incidents, and any necessary reports in compliance with licensing guidelines

Perform housekeeping duties

Prepare resident meals, including planning the monthly resident menu, prepare corresponding grocery list

Plan monthly activities schedule for the home and ensure that it is followed

Develop and implement individualized care plans for each resident, focusing on their specific needs and lead monthly resident meeting

Plan and execute quarterly emergency preparedness drills

Collaborate with social workers, state licensing consultants and other professionals involved in the residents care, coordinate doctor appointments, medication refills and transportation

Attend and successfully complete required staff trainings

Experience:

1 year experience working in home care/adult foster care/nursing care or related field working with vulnerable adults who need daily assistance

High School Graduate or GED equivalent

Valid driver's license with clean driving record

Strong communication and interpersonal skills to effectively interact with residents, families, professionals and staff

Ability to provide a safe and supportive environment for residents

Flexibility to adapt to changing situations and handle challenging scenarios

Applicants must be in overall good physical health and provide results of a physical exam. Must pass background check.

Completion of an approved direct care worker training program
